Focused primarily upon the life of Sayuri, a part-time waitress at a coffee shop. Whilst living a seemingly normal life, Sayuri is in fact able to perceive spirits and is in possession of a variety of spiritual abilities. In the midst of her work as a waitress, Sayuri helps various customers to remediate problems in their lives with the use of her spiritual abilities.

As the movie progresses, Sayuri becomes more active in the practice of exorcism and a friend begins to work alongside her in order to help people resolve problems that are with a spiritual origin, such as lost souls who have yet to move on and are causing disturbances to their relatives.

Eventually Sayuri encounters a more powerful type of malevolent spirit, a devil. The viewer is treated to an extraordinary demonstration of the method of exorcism, which is explained thoroughly in the book, The Real Exorcist. Without giving too much away, it will suffice to say that the viewer will learn of the boundless power of God and the manner in which it may be mediated through faithful and spiritually-attuned humans.

The production and cinematography of The Real Exorcist are visually excellent and the movie depicts the most advanced special effects of any HS Pictures film as of yet. The movie likewise features an exquisite musical composition written by Ryuho Okawa, performed by his daughter Sayaka Okawa, who also wrote the screenplay of the movie, and arranged by both Sayaka Okawa and Yuichi Mizusawa.
